I don’t see my place of care listed in the PKD Foundation Centers of Excellence Program. How can my clinic become part of the COE Program?

If your clinic or provider is not currently listed in the PKD Foundation COE Program, it may simply mean that they have not yet applied. The COE Program is a voluntary application-based initiative. We encourage you to share information about the program with your care team—they may be interested in joining. You can use this handout to introduce the program to your provider and encourage them to apply.
